Dr. Melia is an out-of-network provider with all insurance companies, however a physical therapy superbill - or itemized receipt - can be provided upon request. Submit this to your insurance company for direct reimbursement of your out-of-network benefits.
Under Maryland law, patients can be seen directly for physical therapy services without any restrictions.
Under Virginia law, patients can be seen directly for physical therapy services, evaluation and treatment, for 30 days from initial evaluation before a physical referral is needed.
Under California law, patients can be seen directly for physical therapy services, and may continue to receive treatment for 12 visits or 45 days from initial evaluation, whichever occurs first. After which a physician referral/physician signed physical therapy plan of care is required for continued care.
For pilates or wellness/fitness sessions, you do not need a physician referral.
Come to your appointment wearing clothes you can move in and allow access to the area of concern.
Bring any doctor referrals, imaging reports, or other pertinent documents to your first appointment.
In person physical therapy sessions will be 60 minutes. Your first session, or evaluation, may be a little longer due to the injury history, tests and measures, and patient education that takes place when getting started.
Telehealth physical therapy sessions are 45 minutes, although can be extended to 60 minutes upon request.
Pilates and wellness sessions are 55-60 minutes in duration.
